What is Honor My Decisions?
Some health crises strip us of our most basic right: to speak for ourselves. Honor My Decisions helps ensure your values and medical treatment preferences are known and honored when you are too sick to speak for yourself.

With Honor My Decisions you can:

Your personal URL (which is also stored in a QR code on a wallet card you can print from within the app) provides secure, easy, 24/7 access to your medical wishes for your physicians and healthcare decision-maker.

You may also share your advance care plan with friends and loved ones. Simply add people as Contacts and select what you wish them to have view-only access to. They’ll receive an email with a link to only those parts of your advance care plan.

Is there a fee to use Honor My Decisions?
No. Honor My Decisions is currently offered through medical practices in the United States.

Who can use Honor My Decisions?
An Honor My Decisions account can be established by anyone aged 18 or older who obtains it through their healthcare provider.

What do I need to use Honor My Decisions?
You need access to a computer or mobile device connected to the internet and an up-to-date browser. Supported browsers include: For the best experience possible, we recommend using either Chrome or Firefox.

How do I sign up for Honor My Decisions?
To sign up for Honor My Decisions you need an activation code. This code allows you to create your username and password, which you will then use to log in to Honor My Decisions. You will receive your unique activation code during your appointment with your healthcare provider. You have 30 days from the date of your appointment to use the code before it expires.

From your computer or mobile device, go to app.honormydecisions.com and follow the instructions to use your activation code.

Can I get a new activation code if I have lost it or let it expire?
Yes. If your activation code has expired, please contact your healthcare provider.

My activation code does not work. What should I do?
For your security, your activation code will no longer work once you’ve used it to create your account. If you don’t use the code within 30 days, it will expire. If you have difficulty using your activation code prior to its expiration date, please contact your healthcare provider to request a new activation code.

Can I access Honor My Decisions from my mobile device?
The Honor My Decisions application is designed for desktops, laptops, and tablet-sized devices. While you may use it from any device with access to the internet, the Honor My Decisions application is not currently optimized for devices with smaller screens such as phones.

What is an advance directive?
An advance directive is a living will (a document outlining your medical wishes) and a designated healthcare decision-maker (someone you’ve chosen to make medical decisions on your behalf when you can’t) that together ensure that doctors and emergency professionals know what medical treatments you do and do not want.

Do I really need an advance directive?
You can’t predict if or when you’ll need emergency medical care. If you can’t speak for yourself and you haven’t shared with your loved ones and doctor your medical wishes, you may not get the care you want. You may also unintentionally cause family disagreements or unwanted financial hardships. It’s better to make your wishes known.

Is an advance directive location-specific?
While many states do recognize legally-completed advance directives from other states, it is considered a best practice to have an advance directive for any state you commonly spend a lot of time in. It is critical though that the advance directives reflect the same medical wishes and decisions. For example, if you live in Boston, but winter in Phoenix, you should ideally complete an advance directive for Massachusetts and Arizona. Honor My Decisions makes it easy for you to create advance directives legal in the states that are important to you while ensuring your decisions are the same in each of them.

Is there a form to fill out or is it up to me?
Honor My Decisions provides a structured way for you to record your medical decisions through easy-to- use questionnaires. When you are ready to make your decisions legal, you simply select a state or U.S. territory and we apply your answers to that state's recommended official advance directive form (or forms — some states have a separate form for your living will and healthcare decision-maker while other states combine these decisions into one form). We highlight on the form any questions that still remain for you to answer. When you are satisfied with the form, you can sign and have it witnessed or notarized.

What if I change my mind?
You can change your decisions at any time. Simply log into the app, edit what you want, and save. The new version will replace the old version. You will have to re-sign any documents that require signatures if you change them. You may also delete any of your documents at any time. It’s a good idea to review the decisions you’ve made in your advance care plan at least once a year as well as whenever your health or other circumstances change.

Can someone else change my advance directive?
Doctors and people you’ve shared your advance directive with can only view it, they can’t edit it. Only you can make changes to your materials and that can only be done through the app itself.

How do doctors access my advance directive?
When you create your account you’ll be assigned a unique identification code that is linked to your records. This code is contained in both a QR code printed on a wallet card that you can print from within the app and a personal URL. With that QR code any medical professional can quickly and easily pull up your advance directive and learn your wishes instead of having to track down someone who knows if you have an advance directive and asking them to find and deliver it to the treating physician.

When you create your advance directive you can also proactively share it with your family doctor and/or other physicians by adding them as a Contact and sending them an email link.

Yes. Honor My Decisions is protected by advanced encryption technology, so that only you and the people you have shared with can view your information. Be sure to keep your password private.

Honor My Decisions logged me out. What happened?
For your security, you are automatically logged out after 15 minutes of inactivity on your device. We recommend that you log out of Honor My Decisions if you need to leave your device for even a short period of time.

Please note that our support team can only assist with problems relating to the use of the Honor My Decisions app, not issues related to your personal computer, web browser, or your advance care planning decisions. Contact your provider’s office if you have questions about the contents of your advance care plan or how your unique combination of medical conditions may impact your advance care planning.
